Providing damage reports to Emergency Management allows for better situation awareness.  In the event of a declared disaster, it allows us to better guide recovery agencies to affected areas.  If there are public assets damaged, it will assist the local government in making repairs.

Submission of this form does not guarantee any type of financial assistance. This form is for information purposes only and may be used when submitting county-wide damage assessments or incidents to the State of Florida and FEMA. Information provided on this form may be considered a public record under Chapter 119, Florida Statutes, and may be subject to public disclosure.

  1. Submitting this report does not automatically report such damage to your insurance carrier or FEMA. If you wish to report to your insurance company, do so by calling your agent or the claims number on your insurance documents.
